Job summary

Fundamental is seeking a Social Services Director to plan, develop, organize, implement, evaluate and direct the facility's social service programs. The role requires a current Social Work license, at least one year in human services within a medical setting, and strong organizational and leadership abilities.

Qualifications

• Must have a current Social Work license, in good standing with the state.
• Must have at least one year experience in a human services field in a hospital, skilled nursing care facility or other related medical facility.
• Must be able to organize and prioritize many tasks effectively.
• Ideal candidate must be a skilled communicator, director and motivator.
• Reliable transportation required.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct the facility's social service programs.
• Develop and implement policies for identifying medically-related social and emotional needs of patients/residents.
• Assist in providing solutions to social and practical environmental problems, including seeking financial assistance and discharge planning with community agencies.
