Job summary

CHRISTUS Health Rehabilitation Hospital in Longview, TX seeks a Licensed Case Manager to join our inpatient rehab team. You will coordinate interdisciplinary care, manage discharge planning, and communicate patient progress to families and the care team.

Licensure as RN, Social Worker, Respiratory Therapist, PT, OT, or SLP is required; CCM/ACM/RN-BC/ARN certifications are preferred. A minimum of 2 years in inpatient social work or case management is preferred.

Qualifications

• Education & Licensure: Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Certifications: CCM, ACM, RN-BC, ARN preferred.
• Experience: Minimum of 2 years in social work or case management in an inpatient setting preferred; acute rehab experience strongly preferred.
• Skills: Strong oral and written English communication; proficiency in Microsoft Office.

Responsibilities

• Coordinate interdisciplinary care plan and discharge planning within the inpatient rehabilitation setting.
• Serve as liaison among patients, caregivers, and healthcare team for timely care transitions.
• Document care activities in CMS-compliant IPoC and other records.
• Schedule family conferences and communicate progress and discharge plans with caregivers.
• Coordinate weekly interdisciplinary team conferences to monitor goals and outcomes.
• Review CMG and communicate findings to HIM team.
• Perform other duties as assigned.
