Team Overview

Central Maine Medical Center (CMMC) in Lewiston is the flagship facility of Central Maine Healthcare. It is a 250‑bed, not‑for‑profit, Level III Trauma Center offering comprehensive inpatient and outpatient services, including 24‑hour emergency care and medically supervised orthopedic rehabilitation programs. The rehabilitation facilities are part of an all‑in‑one, single‑floor orthopedics unit designed to support patients in the initial stages of recovery and help them return home sooner.

How You'll Contribute

• Coordinate patient care to ensure appropriate utilization of resources and compliance with regulatory and payer requirements.
• Provide comprehensive case management through assessment, care planning, implementation, and ongoing evaluation of individual patient needs.
• Serve as a patient advocate, supporting care coordination, discharge planning, and continuity of care across the interdisciplinary team.
• Enhance the patient experience, improve quality outcomes, and support cost‑effective care during the rehabilitation stay.
• Collaborate closely with clinical teams, patients, and families to ensure safe, timely, and well‑coordinated transitions of care.

What We're Looking For

• Current Registered Nurse (RN) or Social Work licensure or Healthcare professional licensure as Physical Therapist (PT), Speech Language Pathologist (SLP) or Occupational Therapist (OT).
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., Commission for Case Manager Certification, Association of Rehabilitation Nurses, American Case Management Association, or Board Certification in CM by the ANCC).
• Minimum of 2 years social work or case management experience in an inpatient setting, highly preferred; acute/rehabilitation hospital experience preferred.
