Job summary

Carle Health is seeking a Licensed Behavioral Health Professional to provide comprehensive behavioral health assessment, intervention, and consultation. The role includes crisis evaluation, treatment planning, and coordination with community agencies and care teams.

The LBHP will document thoroughly, meet productivity and compliance standards, and may supervise students or junior clinicians as part of a collaborative care environment.

Qualifications

• Must hold state licensure or be eligible for licensure as LCPC, LCSW, LMFT or Psychologist where required.
• Experience providing behavioral health assessments and treatment planning.
• Knowledge of crisis services and emergency/crisis intervention protocols.
• Ability to document in compliance with regulatory and accreditation standards.
• Familiarity with coordination among community, primary care, courts and managed-care entities.

Responsibilities

• Performs diagnostic assessments, collects information, and determines immediate treatment plan needs.
• Completes treatment plans with patient goals linked to identified needs.
• Provides behavioral health therapy to individuals, families and groups.
• Coordinates with Emergency Response Service and crisis teams for at-risk clients.
• Maintains records per policy and regulatory standards, including Joint Commission and Medicaid.
• Acts as liaison between community resources and patient needs; coordinates treatment goals with multiple agencies.
