Job summary

Carle Health seeks a Licensed Behavioral Health Professional (LBHP) to perform comprehensive behavioral health assessments, interventions, and crisis evaluation under the supervision of the Community Based Therapy Supervisor.

This role involves coordinating treatment plans, providing therapy to individuals and groups, and collaborating with emergency and community partners to ensure patient safety and continuity of care.

Qualifications

• Licensed clinical licensure as LCPC, LCSW, LMFT, or Licensed Clinical Psychologist per IDFPR.
• 2 years of behavioral health experience.
• Use of standard equipment to perform essential functions.

Responsibilities

• Performs diagnostic assessments, collects diagnostic information, and determines immediate treatment plan needs with patients; requests medical consultation when indicated.
• Completes treatment plan with patient, identifying goals connected to needs identified in the assessment.
• Provides behavioral health therapy to individuals, families, and/or groups.
• Coordinates with Emergency Response Service and crisis teams for at‑risk clients.
• Maintains records per UnityPlace policies and applicable regulatory/accreditation agencies.
• Acts as liaison between the community and patient needs; coordinates treatment goals with community agencies, primary care physicians, court system, and managed-care entities.
• Meets clinical productivity standards and complies with documentation and charts.
• May supervise clinicians and students; participates in training and prepares special reports.
