Qualifications

• LSW license or plan to pursue license within 2 years.
• Active licensure inIllinois or equivalently recognized.

• Understanding of chemical dependency treatment and continuum of care.

Responsibilities

• Provides evaluation, counseling and therapeutic services to chemically dependent patients/families.
• Makes treatment recommendations and acts as liaison with treatment network.
• Facilitates programming for court-ordered referrals.
• Assists patient and family with adjustment, motivation for treatment and post-treatment planning.
• Documents per department guidelines and maintains records.
