Job summary

Carle Health is seeking a Licensed Behavioral Health Professional to provide assessments, intervention and consultation for individuals and groups with behavioral health needs in Peoria, IL.

Under supervision, you will plan treatment modalities, coordinate treatment plans, and provide crisis evaluation as needed. The role includes maintaining required documentation and collaborating with community resources to optimize patient care.

Qualifications

• Requires Master's degree in a relevant field (Social Work, Psychology, Counseling or related).
• 2 years of behavioral health experience.
• Licensure eligibility to practice in Illinois is required or applicable license.

Responsibilities

• Performs diagnostic assessments and determines immediate treatment plan needs.
• Completes patient-centered treatment plans with achievable goals.
• Provides behavioral health therapy to individuals, families, and groups.
• Coordinates with Emergency Response Service and crisis teams for at-risk clients.
• Maintains compliant records per policies and regulatory oversight.
• Acts as liaison between community resources and patient needs to coordinate care.
• Meets clinical productivity standards and documentation standards.
• May supervise clinicians or students in training.
