• Performs diagnostic assessments and develops treatment plans with patients.
• Maintains records per policy and regulatory standards.

Responsibilities

• Performs diagnostic assessments and sets immediate treatment needs.
• Completes treatment plans with goals linked to assessments.
• Provides behavioral health therapy to individuals, families and groups.
• Coordinates with Emergency Response Service and crisis teams for high‑risk clients.
• Maintains documentation per policy and regulatory bodies.
• Acts as liaison between community and patient needs; coordinates goals with other agencies.
