Job summary

Wedgewood Healthcare and Rehabilitation in Lakeland, FL is seeking a Licensed Practical Nurse (LPN) for full-time shifts. You will administer medications, support physicians, monitor residents, and provide direct nursing care in a long-term care setting.

The ideal candidate holds Florida LPN licensure, CPR certification, at least 2 years of LTC experience, and demonstrates strong assessment and teamwork while maintaining compliant documentation and compassionate resident care.

Qualifications

• Graduate of an accredited school of nursing.
• Valid LPN license in Florida.
• CPR certification required.
• Minimum of 2 years of experience in long-term care or related settings.
• Strong clinical assessment, critical thinking, and teamwork skills.

Responsibilities

• Administer and document medications and treatments accurately and on time.
• Assist physicians and healthcare professionals with diagnostic procedures and treatments.
• Monitor and report residents’ vital signs, symptoms, and overall condition.
• Provide direct nursing care, including wound care, IV therapy, and other clinical procedures.
• Assist with admissions, discharges, and transfers of residents.
• Maintain accurate nursing documentation in compliance with state and federal regulations.
• Communicate effectively with residents, families, and the interdisciplinary care team.
• Ensure adherence to infection control procedures and facility policies.
• Respond promptly to emergencies and implement appropriate interventions.
• Support CNAs and nursing staff in delivering high-quality patient care.
• Participate in care plan meetings and contribute to resident assessments.
• Educate residents and families on health conditions, medications, and self-care techniques.
• Perform other duties as assigned.
