Job summary

Excelsior Care Group is seeking a dedicated Licensed Practical Nurse (LPN) to join Wedgewood Healthcare and Rehabilitation in Lakeland, FL. This full-time role offers all shifts and a pay rate of up to $33 per hour.

You will administer medications, assist with procedures, monitor residents, and collaborate with the care team. The ideal candidate has a Florida LPN license, CPR certification, and at least 2 years in long-term care, with strong clinical assessment and teamwork skills.

Qualifications

• Graduate of an accredited nursing program.
• LPN license valid in Florida.

Responsibilities

• Administer and document medications and treatments accurately and on time.
• Assist physicians with diagnostic procedures and treatments.
• Monitor and report residents' vital signs, symptoms, and condition.
• Provide direct nursing care including wound care and IV therapy.
• Assist with admissions, discharges, and transfers of residents.
• Maintain accurate nursing documentation per regulations.
• Communicate with residents, families, and care team.
• Ensure infection control procedures and facility policies are followed.
• Respond to emergencies and implement interventions.
• Support CNAs and nursing staff in patient care.
• Participate in care plan meetings and resident assessments.
• Educate residents and families on health conditions and medications.

Key Responsibilities:

• Administer and document medications and treatments accurately and on time.

• Assist physicians and healthcare professionals with diagnostic procedures and treatments.

• Monitor and report residents’ vital signs, symptoms, and overall condition.

• Provide direct nursing care, including wound care, IV therapy, and other clinical procedures.

• Assist with admissions, discharges, and transfers of residents.

• Maintain accurate nursing documentation in compliance with state and federal regulations.

• Communicate effectively with residents, families, and the interdisciplinary care team.

• Ensure adherence to infection control procedures and facility policies.

• Respond promptly to emergencies and implement appropriate interventions.

• Support CNAs and nursing staff in delivering high-quality patient care.

• Participate in care plan meetings and contribute to resident assessments.

• Educate residents and families on health conditions, medications, and self-care techniques.

• Perform other duties as assigned.
