Job summary

Tenacious Health and Wellness LLC in Whitehall, OH is seeking a Licensed Independent Social Worker (LISW) to join our growing behavioral health team. You will conduct psychosocial assessments, develop treatment plans, and provide individual, group, and family therapy in a collaborative, mission-driven environment.

You will document care in EHR, coordinate with multidisciplinary teams, and connect clients with community resources while maintaining professional boundaries and ethical standards.

Qualifications

• Active LISW license in Ohio.
• MSW degree from an accredited program.
• Experience in clinical social work or related setting preferred.
• Knowledge of CBT/DBT and trauma-informed care.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently while maintaining professional boundaries.
• Proficiency with EHR and clinical documentation.

Responsibilities

• Conduct comprehensive psychosocial assessments to evaluate client needs, strengths, and goals.
• Develop and implement individualized treatment plans in collaboration with clients and care teams.
• Provide individual, group, and/or family therapy and counseling services.
• Maintain accurate, timely clinical documentation in compliance with regulatory and agency standards.
• Connect clients with community resources, support services, and referral networks.
• Monitor client progress and adjust treatment plans as clinically appropriate.
• Collaborate with multidisciplinary teams to ensure holistic, integrated care delivery.
