Independence Home Health LLC is seeking a dependable, compassionate Liaison to join our team. The role focuses on building strong relationships with clients, families, caregivers, and our office staff while coordinating services to meet each client’s needs.

Responsibilities include coordinating admissions, working with the Program Manager and field staff, supporting both Support Services and Personal Care programs, and delivering outstanding customer service during business hours.

Qualifications

• High school diploma
• Minimum 6 months of experience with a regulated personal care agency
• Current CPR certification
• Ability to supervise and direct care team members
• Excellent verbal and written communication skills
• Strong interpersonal skills with delegation and tact
• Proficient with Windows-style applications and role-specific software
• Excellent time-management and organizational skills
• Maintain Indiana driver’s license and access to reliable vehicle
• Eligible for Medicaid programs
• Background screenings including driving record, criminal checks, and Sex Offender Registry
• Availability 8:00 AM–4:30 PM with some evenings; in-person or remote as approved

• Coordinate with Program Manager, Scheduling Coordinator, Biller, field staff, and caregivers
• Support both Support Services and Personal Care programs
• Help coordinate services and respond to client and caregiver needs
• Maintain positive relationships with clients, families, caregivers, and partners
• Supervise and provide direction to care team members as needed
• Communicate effectively with the care team to address client needs
• Provide ongoing support, follow-up, and exceptional customer service
• Assist with identifying and coordinating additional services or resources when appropriate

NOW HIRING: SUPPORT SERVICES & PERSONAL CARE LIAISON

Independence Home Health LLC is looking for a dependable, compassionate, and customer-service-driven Liaison to join our team!

The Support Services & Personal Care Liaison will help build strong relationships with clients, families, caregivers, and our office team while coordinating and supporting services that meet each client’s needs.

Position Qualifications & Responsibilities

• High school graduate
• Minimum of six (6) months of experience with a regulated personal care agency or similar company
• Maintains current CPR certification
• Ability to supervise and direct care team members
• Excellent verbal and written communication skills
• Strong interpersonal skills with the ability to delegate and communicate tactfully with clients, families, staff, and the community
• Proficient with Windows-style applications and software programs specific to the role
• Excellent time-management and organizational skills
• Ability to mentor and support agency staff
• Maintains a valid Indiana driver’s license as required by state and/or agency policy
• Maintains access to a reliable, insured automobile in good repair that meets state and/or agency requirements and is capable of fulfilling staff and/or client transportation needs while on duty
• Eligible to participate in Medicaid programs
• Must agree to required background screenings, including driving record, county/federal criminal background checks, national criminal search, and Sex Offender Registry check
• Available 8:00 AM–4:30 PM and some evenings for required meetings
• Available during regular business hours in person, by phone, or through another agency-approved method of communication

Key Responsibilities

• Coordinate and collaborate with the Program Manager, Scheduling Coordinator, Biller, field staff, informal caregivers, and other individuals or entities involved in the client’s care
• Support both Support Services and Personal Care programs
• Help coordinate services and respond to client and caregiver needs
• Maintain positive relationships with clients, families, caregivers, and community partners
• Supervise and provide direction to care team members as needed
• Communicate effectively with the care team to help ensure client needs are addressed
• Provide ongoing support, follow-up, and exceptional customer service
• Assist with identifying and coordinating additional services or resources when appropriate
