Job summary

Genesis Healthcare in Rio Rancho, NM is seeking an LPN to lead wound care initiatives and collaborate with a multidisciplinary team to improve patient outcomes. The role reports to the Director of Nursing with a dotted line to the Nursing Home Administrator and includes coordinating wound rounds and staff training.

You will oversee skin/wound management, serve as Surface Specialist, and work toward completing wound care certifications while advancing in a supportive care environment.

Qualifications

• LPN required with wound care focus.
• Completion of Skin Health Team Lead orientation within 90 days of hire.
• Must complete Wound Basics within 90 days of hire.
• Commitment to Advanced Wound Training and national wound certification within 1 year of hire.

Responsibilities

• Report to the Director of Nursing with dotted line to the Nursing Home Administrator.
• Collaborate with interdisciplinary team to communicate with families, responsible parties, and patients regarding skin and wounds and changes in condition.
• Coordinate and lead weekly wound rounds and post-meet-ups for follow-up discussions.
• Train staff in skin and wound care, aseptic technique, measurement, wound types, skin checks, turning and positioning, treatments and interventions.
• Coordinate readiness for admissions with skin/wound needs with SHTL and DON.
