Qualifications

• LPN license required or eligible.
• Completion of Skin Health Team Lead orientation within 90 days is required.
• Wound care training or certification within 1 year is encouraged.

Responsibilities

• Lead the Skin Health Team to prevent and manage wounds in the nursing center.
• Report to the Director of Nursing with dotted line to the Nursing Home Administrator.
• Coordinate weekly wound rounds and follow-up discussions with the team.
