Job summary

Lincolnshire Community Health Services NHS Trust is seeking an experienced Speech and Language Therapist to independently manage a specialist caseload, conduct comprehensive assessments and develop, implement, and review personalised treatment plans. You will deliver evidence-based interventions for individuals with complex neurological conditions and communicate recommendations to patients and families.

Responsibilities

• Independently manage a specialist caseload, undertaking comprehensive assessments and developing, implementing and reviewing individualised treatment plans.
• Deliver evidence-based interventions for individuals with complex neurological conditions, including aphasia, cognitive communication disorders, dysarthria, apraxia and dysphagia.
• Assess and manage eating, drinking and communication needs, ensuring recommendations are clearly communicated to patients, families, carers and the wider MDT.
• Take responsibility for the development and management of the Speech and Language Therapy service, identifying opportunities to improve clinical practice and service delivery.
• Work collaboratively within the MDT, providing specialist advice, guidance and training to colleagues to support holistic rehabilitation.
• Contribute to the development of clinical policies, guidelines and service improvements, ensuring practice remains evidence-based and in line with professional standards.
• Monitor clinical outcomes, participate in audits and maintain accurate and timely clinical records.
