Qualifications

• Master's degree in Social Work or Counseling is required.
• At least 5 years of work experience in a related field.
• Current independent Social Work (LISW) or LPCC license to practice in Ohio OR LSW with LISW within 1 year of hire.

Responsibilities

• Patient Care - Age Specific and Culturally Inclusive concepts integrated into patient care.
• Interdisciplinary Collaboration to promote patient/family well-being and coordinate care.
• Psychosocial Assessment to identify needs, safety concerns, and plan interventions.
• Psychosocial Interventions and crisis support to patients and families as needed.
• Resource development and leadership to improve care and programs within the team.
