Job summary

Cincinnati Children’s Hospital Medical Center is seeking a Regular, full-time Social Work professional to join the Psych Intake Response Center. The role includes psychosocial assessments, resource identification, and collaborative care planning with the health care team to support patients and families.

Qualifications

• Master's degree in Clinical Counseling or Social Work.
• 2+ years of work experience in a related job discipline.
• LPCC or LISW License in the state Ohio.
• May require clinical license, certification or training.

Responsibilities

• Psychosocial Assessment- Performs psychosocial assessments taking into consideration family, social, and medical history. Reports suspicions of abuse and neglect to legally mandated authorities for investigations. Completes documentation with accuracy and clarity.
• Resource Support- Collaborates with the health care team to develop a treatment plan and identify resources for interventions for patient/family/caregivers.
• Psychosocial Interventions- Provides psychosocial services to improve emotional coping and adherence to the treatment plan; educates patient/family.
• Interdisciplinary Collaboration- Facilitates communication and coordination among health care team and external agencies to minimize fragmentation of services.
• Collaboration- Partners with patient, family, and health care team to ensure safe transition of care and continuous professional growth.
