Qualifications

Responsibilities

• Patient Care - Age Specific and Culturally Inclusive: integrate age-specific and culturally inclusive concepts into patient care.
• Interdisciplinary Collaboration: liaise within the medical team and community to promote well-being.
• Psychosocial Assessment: assess patient/family needs to identify risks and plan interventions.
• Psychosocial Interventions: provide counseling and support to improve coping and adherence.
• Resource Development and Utilization: build programs and collaborate with community agencies.
• Leadership: mentor staff, participate in recruitment, and oversee clinical activities.
• Compliance: ensure practice aligns with regulations and professional standards.
