Job summary

SnapCare in Charleston, WV is seeking an RN for the Day shift to provide comprehensive bedside care to patients in a critical illness rehabilitation setting.

You will perform assessments within 8 hours of admission, develop care plans, and collaborate with families and the care team to meet teaching goals.

This role requires at least two years of bedside experience and current BLS; ACLS is required for CIRH assignments, with opportunities for professional growth.

Qualifications

• Minimum two years of bedside clinical experience.
• BLS certification required before first shift.
• ACLS required prior to first shift (CIRH only).

Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission.
• Develop and implement individualized patient care plans.
• Collaborate with patients, families, and interdisciplinary teams to identify teaching needs and goals.
• Provide patient information and status updates for team conferences.
• Maintain accurate documentation of assessments, interventions, and patient responses.
• Initiate discharge planning from day of admission and provide patient teaching.
• Supervise LPNs and CNAs; assume charge responsibility after orientation if requested.
