Job summary

Signature HealthCARE of Roanoke Rapids is seeking a dedicated LPN/LVN to provide direct nursing care, manage medications, and support residents’ daily needs. You will supervise nursing assistants, ensure safety, and participate in care planning within a supportive, team-based setting.

The role requires an active NC license, 1 year nursing experience (long-term care preferred), and familiarity with EHR systems and MS Office.

Qualifications

• Active LPN/LVN license in NC.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory/charge nurse role preferred.
• CPR/BLS certification preferred.
• Familiarity with EHR systems and MS Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents and report changes to physician or responsible party.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
