Qualifications

• Must be a licensed RN in Virginia.
• At least 2+ years of nursing experience in acute or long-term care.
• CPR certification required; must obtain Dementia Practitioner within 1 year.
• Strong attention to detail and ability to relate to residents and staff.

Responsibilities

• Rounds on assigned residents twice per shift to ensure safety and care plans.
• Administers medications and records in EMR; tracks changes in condition.
• Maintains resident charts, care plans, and daily skilled notes.
• Collaborates with physicians, families, and staff as needed.
• Oversees CNAs, assists with ADLs, and ensures timely vital signs.
• Follows infection control, safety standards, and incident reporting.
• Participates in performance improvement initiatives and trainings.
