Job summary

The RN Case Manager is responsible for providing and supervising primary care to approximately 25-35 patients in their home setting. This includes age appropriate interventions that meet physical, emotional, developmental, psychosocial and educational needs.

The RN Case Manager coordinates all disciplines throughout the episode of care, supervises LPNs and Home Health Aides, and participates with leadership to improve patient and agency outcomes.

Qualifications

• Associates or Diploma in Nursing; BSN preferred.
• Minimum 1 year Medical/Surgical Nursing; Home Health and ICU preferred.
• Current RN license and CPR/BLS certification.

Responsibilities

• Assess, coordinate with disciplines and manage patients' total needs per physician orders.
• Develop and implement care plans; schedule visits based on physician orders.
• Supervise LPNs and Home Health Aides throughout the continuum of care.
• Communicate patient status changes and lab results to the physician as appropriate.
• Ensure CMS/State regulatory guidelines and OASIS documentation are completed.
