Job summary

AMN Healthcare is seeking an RN Case Manager in the San Leandro area to coordinate care, manage resources, and ensure optimal utilization of services. You will perform comprehensive assessments, plan care, and evaluate outcomes while focusing on patient advocacy and discharge planning.

Requirements include a California RN license, AHA BLS, and 2+ years of RN Case Management experience in LTAC/STACH settings.

Qualifications

• CA RN license is required.
• AHA BLS certification required.
• 2+ years RN Case Management exp. in LTAC/STACH, discharge planning across acute care.

Responsibilities

• Coordinate management of care and ensure optimum resource utilization.
• Develop and implement care plans including discharge planning and evaluation.
• Advocate for patients and liaise with IDT to support holistic patient needs.
