Job summary

Oasis Therapy Services is seeking a Physical Therapist to provide one-on-one rehabilitation at Eden Vista Litchfield in Litchfield, Minnesota, a senior living community offering independent living, assisted living, and memory care.

The PRN position emphasizes individualized care, coordination with the care team, and opportunities for flexible scheduling, with competitive compensation and ongoing professional growth.

Qualifications

• Active Minnesota Physical Therapist license.
• Graduate of a CAPTE-accredited Physical Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, independent living, memory care, or rehabilitation experience preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans.
• Provide one-on-one treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention.
• Monitor residents’ progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents and caregivers on exercises, mobility strategies, and equipment.
• Direct and supervise Physical Therapist Assistants as appropriate.
• Complete accurate Net Health documentation and collaborate with the care team.
