ChenMed, a physician-led and mission-driven primary care provider, is seeking a Lead, Primary Care Physician to join our team. This leadership role oversees a panel of patients, influences care delivery, and advances our value-based model across centers.

You will lead SH and TCM meetings, collaborate with center and market leadership, and coach a care team to deliver high-quality, cost-effective care while building a culture of love, accountability, and growth.

Qualifikationen

• Lead PCP role requires strong clinical leadership and accountability for patient outcomes.
• Ability to coach teams and lead care delivery in a value-based model.
• Experience with performance metrics and center-level accountability.

Aufgaben

• Independently provides care for patients with acute and chronic illnesses in older adults.
• Takes full accountability for patient care and outcomes, consulting specialists as needed.
• Engages with hospitalists and coordinates with hospital-based care.
• Leads care teams and coordinates for office-based patients; supports transitions in hospital, SNF, LTC or homebound.
• Leads weekly SH and TCM meetings and supports center leadership as needed.
• Participates in risk and quality management and ensures guideline compliance.
