Job summary

Atrius Health in Chelmsford, MA seeks a Primary Care Advanced Practice Clinician (APC) to independently manage a full primary care panel within a value‑based care environment. You will collaborate with an interdisciplinary team to coordinate care, support shared decisions, and drive quality outcomes.

The role requires active MA NP/PA licensure (or ability to obtain), DEA/MCSR, BLS, and 3+ years of ambulatory primary care experience.

Qualifications

• Active, unrestricted Massachusetts NP or PA license (or ability to obtain prior to start)
• Active DEA and MCSR (or ability to obtain prior to start)
• Current BLS certification
• Minimum of three (3) years of experience independently managing a full primary care patient panel
• Experience in ambulatory primary care with inbox management, test results follow-up, patient messaging and referrals

Responsibilities

• Independently manage a defined panel of primary care patients, providing longitudinal, relationship‑based care
• Perform comprehensive assessments, diagnoses, and treatment of patients in an ambulatory primary care setting
• Manage acute and chronic conditions and supervise patient care plans
• Coordinate care with RNs, pharmacists, population health, behavioral health, and specialists
• Support shared decision‑making and whole‑person care with patients and families
• Provide consultation regarding hospitalized patients within the assigned panel
• Contribute to practice efficiency, quality initiatives, and standardized clinical workflows
