Job summary

The Colorado Department of Public Health and Environment (CDPHE) is seeking a nurse case manager for the Colorado Childhood Lead Poisoning Prevention Program (CCLPPP). You will educate clinics on blood lead testing and reporting, perform testing for children and adults, and guide caregivers to reduce lead exposure across Colorado.

Qualifications

Six (6) years of professional public health nursing or related experience.
Combination of related education (BSN, Healthcare Administration and Management) and two years of experience to equal six years.
Valid Colorado driver license and willingness to travel as needed.

Responsibilities

Serve as nurse case manager within CCLPPP, providing consultation and education to clinics.
Provide blood lead testing to children and adults and train staff in testing procedures.
Notify clinicians of exposure sources and follow up with environmental investigations as required.
Educate clinicians and local public health agencies on reporting requirements and data use for follow-up services.
Ensure data quality and compliance with regulations through quality control.
