Summit Care Inc in Newton, KS is seeking a Physical Therapist to provide therapy services to residents and coordinate care with the rehab team. The role includes evaluation, developing and implementing treatment plans, and discharge planning in accordance with PT guidelines.

You will supervise Physical Therapist Assistants and students, maintain records, ensure compliance with safety and infection control, and participate in quality improvement initiatives.

Formación

• Successful completion of an accredited Physical Therapy Program.
• Valid Physical Therapy License to practice in appropriate state(s).
• Effective written and verbal communication and leadership skills.

Responsabilidades

• Assures execution of doctor’s orders.
• Provides PT screening and evaluation to determine patient status.
• Develops and implements treatment plans with measurable outcomes.
• Oversees and participates in the maintenance of therapy records.
• Maintains appropriate patient schedule, as per established guidelines.
• Documents to maintain quality of medical records and billing requirements.
• Participates in patient, family, and facility meetings; adheres to safety procedures.
• Coordinates and/or participates in training programs for facility staff and residents.
• Initiates new program development to improve residents’ quality of life.
