Qualifications

• Master's degree in an applicable field.
• Oregon license or willingness to obtain licensure in Oregon and Washington.

Responsibilities

• Produce comprehensive Occupational Therapy Evaluations to Parkwood standards.
• Treat clients across ages and abilities continuum.
• Maintain licensure requirements for Oregon and Washington.
• Document using Fusion EMR with CPT/ICD-10 codes and letters of medical necessity.
