Job summary

Upperline Health is seeking a Lead Patient Care Coordinator for our Perimeter office in Atlanta. The role focuses on guiding the clinic team, supporting physicians, and driving patient-centered care within a comprehensive program.

You will lead daily operations, staff onboarding, and patient enrollment in Upperline Plus, while monitoring clinical, operational, and financial KPIs with regional oversight. The position requires strong communication and a compassionate, solutions‑driven approach.

Qualifications

• Experience leading clinic operations and coordinating care teams.
• Strong communication and interpersonal skills; empathetic, professional demeanor.
• Ability to manage staffing, onboarding, and performance in a clinic.
• Proven ability to improve patient experience and drive clinic KPIs.
• Familiarity with care programs and enrollment processes.
• Experience with EMR systems, preferably Athenahealth.

Responsibilities

• Lead daily clinic operations and coordinate care team activities.
• Staffing and onboarding for non-provider clinical roles; manage turnover.
• Oversee patient enrollment in Upperline Plus and engagement efforts.
• Monitor clinic KPIs and collaborate with Regional Manager on improvements.
• Run daily clinic huddles and coordinate scheduling for high‑risk patients.
• Perform monthly audits of clinic operations and EMR documentation.
