Job summary

Tuesday Health is seeking a Palliative Care Advanced Practice Provider to lead clinical care delivery across our remote Ohio program. You will collaborate with the Head of Clinical Delivery and cross-functional teams to ensure high-quality supportive care for patients and families.

The role requires an MSN or PA degree, 5+ years in palliative care or related settings, active OH license, board certification, and DEA license.

Qualifications

  • Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies is required.
  • Active Registered Nurse or Physician Assistant license in the State of Ohio without any board actions.
  • Board certification as an advanced practice nurse by a nationally recognized certifying agency.

Responsibilities

  • Lead internal and joint case rounds to ensure care plan development and execution.
  • Coordinate with Network Providers and Longitudinal Care Providers to align services.
  • Review population-level data and implement interventions within the clinical model.
  • Support provider and clinical staff training to share up-to-date knowledge across the team.
  • Support quality management and quality improvement initiatives.
  • Collaborate with clinical, operational, financial, and technology teams for seamless coordination.
  • Monitor network data and metrics such as utilization, outcomes, and growth opportunities.
  • Represent the company in external forums and events related to palliative care and network management.
  • Stay abreast of trends and regulations in palliative care.

Location: Remote, Ohio

The Role:

The Advanced Practice Provider will serve as a creative and experienced leader of clinical care delivery in partnership with the Head of Clinical Delivery and other Tuesday Health clinicians. They will work with the Head of Clinical Delivery, Head of Operations, market operations team, provider partnership team, and product and clinical teams ensuring the delivery of high-quality supportive care services to our patients and their families.

Key responsibilities of this role may include:

  • Lead internal and joint case rounds of all members to ensure there are no barriers to care, as well as appropriately direct case review to be certain care plan development and execution is appropriate
  • Coordinate with Network Providers and Longitudinal Care Providers to best coordinate services on behalf of the members
  • Review population level data and ensure trends are captured and interventions to respond to trending data and implemented within the clinical model
  • Support provider and clinical staff training on various topics to ensure up to date knowledge sharing across the team
  • Support quality management and quality improvement initiatives as identified
  • Support the performance and quality of our network partners, providing feedback, guidance, support, and corrective actions as needed
  • Collaborate with our clinical, operational, financial, and technology teams to ensure seamless integration and coordination of our network services across the continuum of care
  • Monitor and analyze network data and metrics, such as utilization, outcomes, satisfaction, cost-effectiveness, and growth opportunities
  • Represent our company in external forums and events related to palliative care and network management
  • Stay abreast of the latest trends, innovations, best practices, and regulations in palliative care and network management

A strong candidate will demonstrate the following:

  • Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies
  • Five years clinical experience in palliative care, or hospice, or working with patients with serious illness
  • Active Registered Nurse or Physician Assistant license in the State of Ohio without any board actions
  • Board certification as an advanced practice nurse by a nationally recognized certifying agency.
  • Active Unrestricted DEA license
  • Eligible to maintain adequate malpractice coverage
  • Ability to work with network and longitudinal care providers to support serious illness conversations with members and caregivers (and families)
  • Experience and understanding of value-based contracting with providers
  • Creativity and evidence as being a self-starter
  • Demonstrated leadership skills and ability to inspire, motivate, and influence others
  • Excellent communication, negotiation, relationship-building, and problem-solving skills
  • A passion for supportive care and improving the quality of life of patients with serious illnesses and their families
