Job summary

Sonoma Post Acute in Kentucky seeks a licensed Occupational Therapist to lead patient-centered rehabilitation and drive independence. You will evaluate patients within 24 hours, develop goal-driven plans, and supervise COTAs and staff to ensure high-quality care.

You will collaborate with physicians and the care team, participate in discharge planning, and maintain accurate documentation in EMR systems to ensure smooth transitions and optimal outcomes.

Qualifications

• Bachelor’s Degree in Occupational Therapy (Master’s or Doctorate preferred).
• Active Ohio Occupational Therapist license in good standing.
• Committed to ongoing continuing education.

Responsibilities

• Perform patient evaluations within 24 hours of referral.
• Develop and implement individualized, goal-driven treatment plans.
• Provide hands-on therapy per physician directives and monitor progress.
• Lead and mentor COTAs, aides, and students; collaborate with nursing and interdisciplinary teams.
• Participate in care conferences, utilization reviews, and rehabilitation meetings.
• Maintain accurate documentation in accordance with state acts and payer guidelines.
