Job summary

Mercer Manor Nursing and Rehabilitation seeks a skilled Occupational Therapist to lead expert OT services at our premier skilled nursing facility. You will evaluate residents, develop personalized treatment plans for ADLs/IADLs, and guide evidence-based, occupation-focused interventions with the care team.

Qualifications

• Licensed occupational therapist eligible to practice in Illinois.
• Experience evaluating residents in geriatric rehabilitation.
• Ability to design and implement individualized treatment plans.

Responsibilities

• Perform detailed evaluations of residents' physical, cognitive, perceptual, and psychosocial abilities and design plans to improve ADLs and IADLs.
• Lead occupation-based interventions, adaptive equipment training, energy conservation, cognitive strategies, and environmental modifications.
• Track progress, document outcomes, educate residents, families, and staff, and collaborate with an interdisciplinary team to adjust plans.
