Enhance Rehabilitation is seeking a dedicated Occupational Therapist to join our team. You will evaluate and treat patients in a skilled nursing facility setting, following individualized plans of care and measurable goals.

You will collaborate with facility staff, document evaluations and progress, and supervise Occupational Therapy Assistants while educating patients and families about progress and expected outcomes.

Qualifications

• Illinois OT licensure (active)
• Experience in SNF/skilled nursing settings preferred
• Strong documentation and communication skills
• Ability to develop and monitor plans of care with measurable goals

Responsabilités

• Evaluate and treat patients in SNF following individualized care plans
• Develop plans of care and set measurable functional goals
• Document evaluations, re-evaluations, progress notes, and discharges timely and accurately
• Supervise Occupational Therapy Assistants (OTAs)
• Collaborate with facility staff to ensure high-quality patient care
• Educate patients and families on injury, progress and expected outcomes
