Requires a NYS OT license and a relevant degree; geriatric experience is preferred. Strong communication and ability to work independently and within a team are essential for success.

Qualifications

• The candidate must hold a current unencumbered NYS license as a Registered Professional Occupational Therapist, with a Bachelor’s degree or higher.
• Graduate of an accredited school of occupational therapy or international equivalent.
• Geriatric experience is preferred.

Responsibilities

• Screens residents for rehabilitation therapy needs.
• Completes Occupational Therapy evaluations upon physician order.
• Establishes Occupational Therapy Plan of Care per physician order.
• Provides OT treatment plans and modalities.
• Documents resident progress toward OT goals.
• Discharges residents from OT services when appropriate.
• Supervises OT Assistants and unlicensed personnel.
• Participates in discharge planning with the interdisciplinary team.
• Complies with Medicare Part A PPS case management and regulatory requirements.
