Job summary

TeamCare Medical in New York seeks a Clinical Lead - Nurse Practitioner to collaborate with primary care physicians, oversee patient care, take detailed histories, perform exams, and manage chronic conditions within the NP scope of practice.

You will educate patients and families, document in the Athena EHR, and participate in quality improvement initiatives to enhance outcomes across the care team. Licensure, board certification, and DEA are required.

Qualifications

• Education requirement: NP program acceptable to NYSED
• Experience: 2+ years in clinical nursing (primary care or related).
• Must hold NY NP license and RN license; DEA and board certification.

Responsibilities

• See adult patients at TeamCare site and serve a diverse community.
• Represent Medical Director role when required at location.
• Perform annual wellness assessments.
• Educate patients on medical conditions and comorbidities.
• Conduct routine quarterly assessments for chronic illnesses.
• Provide acute care services and same-day access coordination.
• Manage acute changes with in-person or telemedicine visits.
• Conduct post-hospitalization visits with medication reconciliation and care coordination.
• Document diagnoses and care plans accurately in EHR on visit day.
• Review symptoms and active conditions to guide medical management.
• Supervise care plan formulation and ensure proper delivery by each discipline.
• Act within scope of practice and meet TeamCare competencies.
• Perform additional duties as assigned.
• Schedule: Mon–Fri 8:00AM–5:00PM
• Weekly Hours: 40
