TeamCare Medical is seeking a Clinical Lead - Nurse Practitioner to collaborate with primary care physicians to oversee patient care. See adult patients, perform wellness assessments, manage chronic illnesses and document in the Athena EHR.

The role emphasizes teamwork, education, and quality improvement within a busy clinic. Qualified candidates hold NY NP and RN licenses, DEA certification, and board certification.

Qualifications

• Graduate from a nurse practitioner program acceptable to NYSED.
• NYSED-accepted NP license and NY RN license required.
• DEA certification and AANP/ANCC board certification required.
• Two years clinical nursing practice in primary care or related field preferred.
• Strong communication and documentation skills in EHR systems.

Responsibilities

• NP will be assigned to a TeamCare site and see adult patients.
• Represent the Medical Director role when directed at location.
• Conduct annual wellness assessments and routine quarterly reviews.
• Provide acute care and same-day access for participants.
• Perform telemedicine visits when appropriate and coordinate care.
• Develop comprehensive diagnostic lists and enter accurate codes in EHR.
• Supervise care plan delivery across disciplines and ensure adherence.
• Operate within the NP scope of practice and meet TeamCare competencies.
• Performs other duties as assigned.
