Job summary

Adaptive Home Health, LLC is seeking a dedicated Registered Nurse (RN) - Case Manager in Littleton, Colorado. This full-time position requires strong clinical judgment to deliver quality home health care, particularly in wound care and infusion therapy.

You will conduct patient assessments, manage care coordination, and advocate for patients while benefiting from flexible scheduling and mileage reimbursement.

Qualifications

• Must have a current Colorado RN license.
• 3+ years of nursing experience in a clinical setting.
• Experience and comfort with wound care and infusion therapy.
• Strong time management skills in a high-autonomy environment.

Responsibilities

• Conduct patient assessments and documentation including OASIS.
• Provide high-acuity home health care and carry out plans of care.
• Coordinate services to ensure execution of the care plan.
• Administer medications and provide caregiver teaching.
• Supervise and guide LVNs and HHAs as applicable.
