Job summary

Community Bridges is seeking a Lead Navigator to support housing outreach and case management for individuals experiencing homelessness. The role emphasizes coordination of community services, wrap-around supports, and mentorship of junior navigators.

The Lead Navigator coordinates entry processes, housing referrals, leasing up, and discharge planning within a multidisciplinary team to improve patient outcomes and stability.

Qualifications

• Requires highschool diploma or GED; associates in behavioral health preferred.
• 1–3 years health care experience in crisis settings is preferred but not required.
• BHT certification and a clean 39‑month MVR are required unless otherwise stated.

Responsibilities

• Provide outreach and supportive services to assist getting homeless individuals housed.
• Coordinate RBHA and Non-RBHA services; monitor and facilitate care delivery.
• Mentor Navigator II; onboard and train team; manage coordinated entry and wrap‑around services.
• Work within a multidisciplinary team to improve patient outcomes.
