Job summary

Community Bridges, Inc. (CBI) is seeking a Lead Navigator to assist homeless individuals by providing outreach and wrap-around services. The role acts as a bridge coordinating with RBHA and non-RBHA services, ensuring access to housing and continuum care.

The Lead Navigator is a mentoring figure for Navigator II, overseeing onboarding and program requirements while coordinating entry, referrals, and discharge through a collaborative, multi-disciplinary team.

Qualifications

• Highschool diploma or GED required; associate degree in behavioral health preferred.
• 1–3 years of full-time health care experience with exposure to behavioral/medical crises preferred but not required.
• Minimum of six months of recovery from substance use and/or mental health disorders required.
• Or equivalent combination of education and experience.
• Current AZ Driver\'s License.
• Clear 39-month Motor Vehicle Record.
• Behavioral Health Technician (BHT) per AC policy.
• Arizona Fingerprint Clearance card, site-specific.

Responsibilities

• Outreach and provide supportive services to assist individuals who are homeless.
• Coordinate and monitor RBHA and non-RBHA community-based services.
• Mentor Navigator II to onboard, understand the program, and fulfill requirements.
• Coordinate entry, housing referrals, leasing up, wrap-around services, and discharge.
• Collaborate with a multi-disciplinary team and community providers.
