Job summary

Community Bridges, Inc. (CBI) invites applications for a Lead Navigator.

This role is for someone in recovery with at least 1 year and 90 days of Navigator or Peer Support experience, focused on outreach to individuals experiencing homelessness and coordinating services. You will coordinate RBHA and non-RBHA community-based services, perform outreach, case management, and wrap-around supports, and mentor Navigator II to onboard and meet program requirements.

Qualifications

• Highschool diploma or GED required; associate degree preferred.
• 1–3 years of health care experience with exposure to behavioral and medical crisis situations is preferred.
• 1–3 years of substance abuse/mental health recovery-oriented service is preferred.
• Current AZ Driver's License (Valid and in good standing).
• Clear 39-month Motor Vehicle Record.
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-115 and CBI policies.
• Arizona Fingerprint Clearance card, site-specific.

Responsibilities

• Outreach and supportive services to assist the homeless population.
• Coordinate and monitor RBHA and non-RBHA community-based services.
• Provide ongoing wrap-around supportive services and case management.
• Mentor Navigator II to help onboarding and program requirements.
• Coordinate entry, housing referrals, leasing up, wrap around services, and discharge.
