Job summary

Community Bridges, Inc. (CBI) seeks a Lead Navigator (in recovery) to support individuals experiencing homelessness by delivering outreach and wraparound services.

This role coordinates with RBHA and other community-based providers to ensure seamless access to housing and care resources. The Lead Navigator mentors a Navigator II, leads coordination efforts for housing referrals, entry, leasing, and discharge, and collaborates within a multi-disciplinary team to reduce homelessness and improve

Qualifications

• Requires recovery from AOD use and/or GMH for at least 1 year.
• 1-3 years of full-time health care experience preferred but not required.
• Current AZ Driver's License and clean 39-month MVR.
• Behavioral Health Technician credential per A.A.C. R9-10-115 and site policies.

Responsibilities

• Provide outreach and supportive services to homeless individuals.
• Coordinate and monitor RBHA and non-RBHA community-based services.
• Engage, assess, and deliver wraparound services to clients.
• Mentor Navigator II to onboard and guide program requirements.
• Coordinate entry, housing referrals, leasing up, and discharge processes.

Skills

Recovery from AOD/GMH

1-3 years health care experience

Driver's license

MVR clear (39-month)

BHT/Aide certification

Education

High school diploma or GED
