Job summary

Phoenix Senior Living community is seeking a Lead Med Tech to join our team. The role includes monitoring residents' health, leading care activities, and administering medications while coordinating with wellness services and families.

The Lead Med Tech will support the Wellness Nurse, complete monthly wellness visits and PSPs, and ensure accurate documentation of any changes in clinical care or services. Weekend/evening hours and flexibility are required.

Qualifications

• Must be certified Medication Aide or meet state requirements for medication aide.
• A minimum of one year of experience working with elders or disabled individuals, preferred.
• Must have demonstrated leadership capabilities.
• Knowledge and experience in Assisted Living and Dementia care or Skilled Nursing, preferred.
• Ability to work weekends, evenings and flexible hours for peak service delivery times.

Responsibilities

• Lead Med Tech is responsible for monitoring the health and wellbeing of residents and provides leadership support and assistance with resident care and services.
• Medication care assistance/administration, communication with resident support services and families.
• Maintaining clinical quality assurance while leading, demonstrating and supporting wellness elements.
• Identify ongoing needs and services of residents through assessment/PSP process.
• Ensure proper follow through and documentation for changes in clinical care needs.
• Assist Wellness Nurse with completing monthly wellness visits and PSP to reflect residents monthly.
