Paradigm Healthcare Creekside in Gahanna, Ohio is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. You will provide compassionate, high‑quality care to residents and work under the supervision of a registered nurse or physician.

The role emphasizes adherence to patient plans, HIPAA, and facility policies, with rounds to ensure standards. LTC and rehabilitative experience are preferred, and proficiency with Point Click Care and basic computer tools is desirable.

Qualificações

• Must possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse in the State (OH).
• 2 years of Long-Term Care experience preferred.
• Six months of rehabilitative/restorative nursing experience preferred.
• Strong leadership and organizational skills; professional attitude.
• Proficient in Point Click Care and basic computer literacy.

Responsabilidades

• Provide direct nursing care under supervision of a RN or physician.
• Deliver care to residents per the plan of care and physician orders.
• Follow facility policies and procedures; protect resident privacy and report HIPAA concerns.
• Perform rounds to ensure nursing staff perform to standard.
• Oversee nurse aides and coordinate with other staff to maintain department operations.
