Creekside Care Center, a skilled nursing facility in Mifflin Township, OH, seeks an LPN to join our caring team. You will provide direct nursing care under RN or physician supervision, following the resident's plan of care and all applicable rules and guidelines that govern nursing facilities.

The ideal candidate holds an active Ohio LPN license, with 2 years in long-term care and 6 months in rehabilitative nursing preferred.

Qualifications

• Must possess a current, unencumbered, active license to practice as an LVN/LPN in Ohio.
• 2 years’ experience in Long-Term Care preferred.
• Six (6) months of experience in rehabilitative and restorative nursing preferred.
• Must demonstrate leadership, organizational skills, and maintain a positive attitude.
• Computer literacy with Outlook, Office, and internet.

Responsabilités

• Provide direct nursing care under the direct supervision of RN or physician.
• Deliver care to residents following their plan of care and physician orders.
• Adhere to facility policies, procedures, and applicable regulations.
• Protect resident privacy and report disclosures per policy.
• Make frequent rounds to ensure nursing staff meet standards.
• Oversee nurse aides and coordinate with staff to maintain procedures.

If you agree, Paradigm Healthcare is the place for you!ResponsibilitiesProvide direct nursing care, under the direct supervision of a registered nurse or physician, in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgment.Deliver care to residents following their plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilities.Have knowledge of and utilize the facility’s written policies and procedures that govern the day-to-day functions of the nursing service department.Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.Make frequent rounds of the assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.Oversee nurse aides and communication with and oversight of nursing home staff to ensure that specific policies, procedures, and systems are maintained correctly as it relates to the department.RequirementsMust possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse in the State2 years’ experience in Long-Term Care experience preferred.Six (6) months of experience in rehabilitative and restorative nursing practices preferred.Must be able to demonstrate leadership, organizational skills, and maintain a positive and professional attitudeStrong utilization knowledge of Point Click Care preferredComputer literacy skills (Microsoft Outlook, Microsoft Office, Internet)Paradigm Healthcare was founded on the belief that by empowering each other, we can achieve more and provide better care to every patient we serve.
