Qualifications

Responsibilities

• Coordinate care with the RN Case Manager focusing on psychosocial assessment and intervention.
• Assist patient, family, and healthcare team in transition of care and complex discharge planning.
• Document in the medical record and collaborate with hospital disciplines and community agencies.
• Manage behavioral emergencies beginning in the Emergency Department and continue through continuum of care as a backup to the ED social worker.
