BrightStar Care of Concord, Lexington & Woburn is seeking an experienced Registered Nurse to lead in-home clinical care. You will supervise care staff, develop patient care plans, perform in-home assessments, and ensure high-quality service.

Qualifications

• Active RN license is required.
• CPR certification required.
• 3–5 years of clinical experience with 1 year in home health, primary care, or health facility preferred.
• Strong skills in IV therapy, wound care, and G-tube care.
• Reliable, detail-oriented, and passionate about patient care.
• Must have reliable car/auto insurance.

Responsibilities

• Lead and manage all clinical aspects and care-staff for assigned business
• Develop and implement individualized patient care plans
• Perform in-home assessments and ensure quality care delivery
• Administer medications, IV infusions, and treatments as prescribed
• Provide advanced wound care, including Wound VAC (NPWT) management
• Maintain accurate documentation and ensure regulatory compliance
• Supervise and support nurses, aides, and caregivers
• Coordinate staffing and ensure proper coverage for patient cases
• Collaborate with physicians and healthcare professionals
