BrightStar Care Concord, Lexington & Woburn is seeking Registered Nurses (RN) to join our home health team across Woburn, Concord, and surrounding areas. The role focuses on developing care plans, in-home assessments, and ensuring regulatory compliance while leading a care team.

Qualifications

• Active RN license in the operating state is required.
• CPR certification is required.
• 2–3 years experience in home health, hospice, or skilled nursing preferred.
• Experience with IV infusions and wound care preferred.
• Strong leadership, care planning and risk management skills are expected.

Responsibilities

• Develop and implement patient care plans in collaboration with physicians.
• Conduct in-home assessments and ensure regulatory compliance.
• Oversee home health regulations documentation and reporting.
• Supervise and mentor nurses, aides, and caregivers.
• Provide guidance to ensure high-quality patient care and scheduling.
• Manage staffing for home care cases.
