Job summary

Hammer & NER is seeking a Lead Direct Support Professional to assist the Program Manager with administrative tasks and provide daily support to individuals with intellectual and/or developmental disabilities living in Hammer & NER homes and apartments. You will build meaningful relationships, promote life skills, and help maintain a safe living environment that centers on dignity and person-centered care.

Qualifications

• Must be 18 years of age or older.
• Proficient in using computers and/or smart technology.
• Exceptional interpersonal skills and ability to communicate effectively throughout the organization; must be fluent reading and speaking English.
• Must be cleared by a background check through the MN Department of Human Services.
• Requires a valid driver’s license and a driving record that meets insurance requirements; ability to drive an organizational vehicle.

Responsibilities

• Administrative Support: Promptly schedule medical appointments and promote communication and activities within the community.
• Daily Living Supports: Assist residents with daily living skills and personal cares, maintaining a clean living environment.
• Communication: Respond to communications, use Outlook/Teams/SharePoint, and report issues to the Program Manager.
• Training and Compliance: Ensure trainings are completed by due date and maintain confidentiality and HIPAA compliance.
