Job summary

Hope Network is seeking a Lead Direct Care Professional to oversee and deliver direct care across settings. You’ll model person-centered support, promote independence, and ensure implementation of care plans while mentoring new staff.

You will also participate in on-call responsibilities and provide guidance when supervisors aren’t available, helping maintain a respectful, safe environment for individuals served.

Qualifications

• High school diploma or equivalent required.
• Valid driver’s license required.
• Ability to lift 50 lbs. and pass background checks.
• Ability to obtain CPR/First Aid certification.

Responsibilities

• Lead on shift to oversee, support, and deliver direct care.
• Mentor, train, and orient new staff.
• Assist with on-call responsibilities and provide guidance when supervisors are unavailable.
• Promote independence and implement medical, clinical, therapeutic, and behavioral plans.
• Foster a safe, structured, and supportive environment.
